TUITION AND POLICY AGREEMENT

FINANCIAL OBLIGATIONS:

* Application is confirmed after receipt of completed application and accompanying registration fee. An invitation to enroll in ShulCloud will be emailed beginning in March to all families registered for the upcoming school year. At that time tuition deposit of $300/child ($500 for full day) will be required to secure a spot for the fall.

* Families accepting a position, but withdrawing after July 1st, will be responsible for one full tuition payment, two full tuition payments for an August 1, 2022 withdrawal, 3 full tuition payments for a September 1, 2022 withdrawal and so on. All tuition paid to date is non-refundable. If a family requests a withdrawal, four weeks written notice is required plus the child’s final month’s tuition must be paid. If a child is temporarily withdrawn from the preschool for any length of time, the child’s place will be reserved only upon continued payment of tuition.

Families accepting a position, but withdrawing after July 1st, will be responsible for one full tuition payment, two full tuition payments for an August 1, 2022 withdrawal, 3 full tuition payments for a September 1, 2022 withdrawal and so on. * Tuition will be charged at the member rate for families who maintain full membership status and are current in their Etz Hayim dues when billing for the year is established. Annual financial obligations to Etz Hayim must be kept current throughout the school year. Families who become Etz Hayim members after the start of the school year will receive the member tuition rate for the remainder of their tuition once their first dues payment has been submitted. Members who do not remain in good standing, as determined by Congregation Etz Hayim, are at risk for their tuition reverting to the non-member rate.

LATE FEE:

* Afternoon dismissal times for the half and full day programs are 12:30pm and 5pm, respectively. Pickup after 12:30pm or 5pm, or the designated After Care pickup time will incur a late pickup fee of $15. Charges will be incurred each additional 15 minutes or any part thereof after your scheduled pickup time.

UPDATING STUDENT INFORMATION:

* Parents acknowledge their responsibility for keeping the school advised of any changes in student information, such as phone numbers, home address, work location, emergency contacts, medical updates, and family physician.

5 DAY PRIORITY POLICY

* We try to accommodate all students. Sometimes we are able to enroll a student 3 days into a 5 day class. However, in the case of full classroom capacity, a 5 day applicant will receive priority over a 3 day applicant. Partial enrollments are confirmed after August 1, 2022.

* For themselves, their agents and their children, parents, by the electronic submission of this form, waive any and all claims of every nature and description they, their agents or their children may have in the future against Congregation Etz Hayim Preschool, its administrators, teachers, employees or agents related to the Preschool unless such claims arise from the intentional wrongful acts or gross negligence of those parties otherwise released.
